Dual DiagnosisHelping a Family Navigate a Challenging Transition

Story provided by April Meredith, Statewide Information & Referral Coordinator, TN Disability Pathfinder

Every day, parents, caregivers, educators, and service providers encounter moments that feel bigger than the resources they have on hand. These aren’t emergencies, they’re crossroads. And often, what families need most is someone who understands the landscape well enough to help them take the first step with confidence. This month’s story is a reminder that timely information, paired with genuine listening, can completely change a family’s outlook.

When Middle School Special Education Teacher Daryl spoke with a concerned mother of a student with multiple disabilities, he realized the family needed services beyond the learning environment.  He quickly recognized that this was not a simple classroom concern but a layered situation involving developmental, emotional, and mental health factors all happening at once. Now that her son is hitting puberty, the mother is noticing physical, emotional, and behavioral changes that are difficult for any parent of a teenager, yet even more challenging because of her child’s dual diagnosis of intellectual disability and mental health needs. These overlapping changes left her uncertain about what was typical, what was cause for concern, and where she could even begin seeking support.

After the parent-teacher conference, Daryl reached out to Tennessee Disability Pathfinder for ideas and guidance. “I could tell she is overwhelmed and unsure where to get support, information, and resources to help her and her child,” Daryl said. “I want to know a good place to start that I can tell the family.”

Pathfinder’s Information & Referral Coordinator provided a tailored list of resources for Daryl to share with the family and encouraged him to pass along Pathfinder‘s contact information in case the family wanted direct navigation assistance.  Because the situation involved both developmental and mental health needs, the coordinator selected resources that addressed the complexity of the dual diagnosis rather than relying on a one-size-fits-all recommendation.

What seemed like a small step, one conversation, one email, one connection, made a noticeable difference. Daryl was grateful for the immediate suggestions and for having a clear, reliable starting point. He left the conversation grateful for clear next steps and confident that other students’ families can also rely on Pathfinder when they need help connecting to community services.
